The square takes its name from the Church of San Cosimato, dating back to the 10th century. The area where the square stands is located on the edge of what was the Trastevere district until the end of the 19th century, and as such, it appeared almost like a countryside area, characterized by a large, triangular-shaped clearing, known as Prati di San Cosimato, bordered by trees.

Around the 1880s, the San Cosimato district arose in this area, radically changing the square, while maintaining its characteristic triangular shape and large dimensions, around which five-six-story buildings were built.

The area became home to an important market, which still exists today. Along with the square, it underwent a series of renovations that changed its appearance at the beginning of the 21st century.
